Registering as an NHS Patient at the Practice Please print out the attached GMS1 form, complete it and sign it by hand. Send it by post to the Assistant Practice Manager or deliver it by hand to the surgery. You will be sent a new patient questionnaire which you need to fill in and return to the surgery to complete the registration process. |
